
"Normal" Labs Don't Explain Your Symptoms. But I Do.
You've been here before.
The waiting room. The paper gown. The bloodwork order that felt, this time, like it might finally give you an answer.
And then the call. Or the portal notification. Or the provider who walks in already talking about something else.
"Everything looks normal."
You smiled. Said thank you. Drove home.
And cried in your car.
Because normal doesn't explain the afternoon crash that has you counting the hours until you can lie down. It doesn't explain the hair in the shower drain, the weight that won't move no matter what you try, or the brain fog so thick some days you lose the thread of your own sentence. It doesn't explain feeling like a stranger in your own body.
So many women have been told they're fine so many times that they start to believe it. Here's what's actually going on — and why "normal" was never the right question.
The Problem With "Normal"
The standard lab panel your provider orders is designed to catch disease. It's built around reference ranges — the values that capture the middle of a broad population. If your number falls inside that range, you're flagged as normal and the conversation ends.
But here's what that system can't tell you: normal is not the same as optimal.
Take TSH, the standard thyroid screening test. Most labs consider anywhere from about 0.4 to 4.5 mIU/L "normal." A result sitting right at that upper edge is still read as normal — even though a TSH in that range is associated with fatigue, weight gain, constipation, and brain fog in many women, and often marks the early, subclinical stage of an underactive thyroid. You can be at the edge of the range, deeply symptomatic, and still be sent home without answers.
The range was built to rule out how sick you are. It was never built to explain how you feel. Those are two very different questions.
What Standard Testing Misses
When a patient comes to me, I'm not just looking at TSH. I'm looking at the full picture — because thyroid health, hormone balance, and how your body actually functions don't live in a single number.
Here's what I look at that most standard panels skip:
Free T3. T3 is the active thyroid hormone your cells actually use. Your body converts T4 (the storage form) into T3, and if that conversion isn't happening efficiently, you can have a completely normal TSH and still be functionally low on usable thyroid hormone. Most standard panels don't include Free T3 at all.
Reverse T3. Under physical or emotional stress, illness, or inflammation, your body can shift T4 conversion toward reverse T3 instead of active T3 — a pattern researchers describe as part of the body's stress response. The clinical significance of reverse T3 testing is genuinely debated in endocrinology, and I don't use it in isolation to make a diagnosis. But in the right clinical context, alongside TSH, Free T4, and Free T3, it's one more piece of the pattern — not a stand-alone verdict.
Thyroid antibodies (TPO and TgAb). Hashimoto's thyroiditis is an autoimmune condition that attacks the thyroid. You can have it for years before TSH ever shifts outside the standard range. Antibodies can flag what's happening long before the damage shows up on a routine panel.
Ferritin. Low ferritin is one of the most common — and most overlooked — causes of fatigue and hair loss in women. Your hemoglobin can be completely normal, so you're told you're not anemic, while your iron stores are quietly depleted. Hemoglobin and ferritin are not the same test, and they don't tell the same story.
Cortisol and adrenal function. Chronic stress affects cortisol patterns, and cortisol patterns affect how well your body converts thyroid hormone. Your thyroid and your adrenal system are in constant conversation. Testing one without the other gives you half the picture.
Vitamin D, B12, and magnesium. These aren't fringe extras. They're cofactors your body requires for hormone production, conversion, and basic cellular function — and deficiencies in any of them can produce symptoms that look a lot like thyroid disease. They're almost never included in a standard panel.
Why This Keeps Happening
This isn't a story about bad providers. Most clinicians are working inside a system that doesn't give them the time, the training incentives, or the tools to go deeper.
A standard primary care visit runs, on average, less than twenty minutes. In that window, a provider is managing medications, answering questions, documenting for the electronic record, and making decisions about care. The system was built for efficiency — and efficiency means ruling out what's dangerous, not investigating what's wrong.
The gap between "not sick" and "actually well" is where millions of women live. That gap has a name. And it's where I work.
What Functional Medicine Does Differently
Functional medicine doesn't start with your labs. It starts with your history, your symptoms, your life. The labs come after, and they're chosen to answer specific questions rather than run a general screen.
When I work with a patient, I want to understand:
When did this start — and what was happening in your life at the time?
What makes it better? What makes it worse?
What have you already tried, and what happened?
What does a good day look like — and how long has it been since you had one?
These aren't soft questions. They're diagnostic. The pattern of symptoms across time tells me where to look and what to test. By the time I review labs, I already have a working hypothesis.
That's a different kind of medicine. It takes longer. It asks more of both of us. And for women who've spent years being told their labs are normal, it's often the first time a provider has actually listened all the way through.
You Are Not the Problem
The appointments that went nowhere. The tests that came back normal. The quiet voice that started to wonder if maybe everyone else was right.
You are not the problem. You're a person whose body is trying to tell you something — and the tools being used to listen weren't built to hear it.
